If you purchased a paid subscription from Sirius XM that was marketed as a “Lifetime plan” or “Lifetime Subscription,” AND you have one or more “Inactive Lifetime Subscriptions”, then you must complete and submit a Claim Form either online or via mail so that it is received no later than January 12, 2021, in order to receive either of the forms of benefit under the Settlement. Claim Forms received after this deadline will be deemed untimely and may not be accepted.

Please Note: That if you hold an "Active Lifetime Subscription", according to Sirius XM’s records, you automatically qualify for the Settlement’s benefits, assuming the Settlement is approved by the Court, and you do not have to file a Claim Form.
Remember, All Claim Forms must be submitted online or received via mail no later than January 12, 2021.
